Prior to your appointment, you are responsible for contacting your insurance to be sure that your appointment and the practitioner you are scheduled to see are covered by your policy. You are responsible for paying any charges that your insurance policy does not cover.

Because of the large number of insurance carriers and types of policies, our office is unable to tell you whether your visit or procedure will be covered. Please call your insurance company for that information. The phone number is probably on the back of your insurance card.
